Abstract Objective To estimate the predictive value of urinary neutrophil gelatinase-associ⁃ ated lipocalin(uNGAL)on detection of acute kidney injury(AKI)in critically ill patients. Methods A total of 162 participants from ICU of Shanghai Baoshan Hospital were enrolled in the study. The rele⁃ vant clinical data were collected. In 0, 24 , 48 h uNGAL, serum creatinine were detected. Acute physi⁃ ology and chronic health evaluation Ⅱ(APACHEⅡ)and sequential organ failure assessment(SOFA) were assessed and compared between AKI and non-AKI patients. Diagnostic significance of uNGAL was evaluated by a receiver operating characteristic(ROC)curve. Results There was significant differ⁃ ence of uNGAL and serum creatinine levels between AKI patients and non-AKI patients(P<0.05). In patients among grade AKI 1~3, the uNGAL value and SOFA score were increased with the in⁃ crease of grade. The area under the ROC curve of uNGAL was 0.838, the sensitivity and specificity were better than that of serum creatinine. Conclusion uNGAL is helpful for early diagnosis of AKI, and may be predictive for the severity of AKI.
